Advancing Health for Tribal Nations

For too long, US trust and treaty obligations to Tribal Nations went unmet. Nowhere was this more threatening to the lives and health of their citizens than in the discretionary budget for the Indian Health Service (IHS). Even when funding levels were near adequate, government shutdowns meant the loss of access to doctors and medicine and greater stress for communities, families and elders. It also meant that essential workers in emergency services, hospitals and nursing homes worked without pay.  

In 2022, we saw the opportunity to end the unnecessary disruptions that threated lives and health outcomes. Through advance appropriations, IHS could receive its annual funding allocation a year early. Working with the National Indian Health Board, we orchestrated a national strategy with a coalition of Tribal organizations and elders to raise these concerns with lawmakers.  

In four months, the strategy proved successful, and the first advance appropriation for IHS was enacted as part of the FY23 spending bill. Advance appropriations continued with $5.1 billion in FY24 funds made available to IHS on October 1, 2023.
